Request for Change in Instructional Delivery
The Texas Education Agency allows school districts to ask parents to commit to either in-person instruction or remote instruction during the COVID pandemic. FSHISD is requesting that parents choose either in-person or remote instruction for an entire grading period and if a switch is needed the request is to be turned into campus administration two weeks prior to the beginning of the next grading period. Please see the steps below to complete this process.

- Complete the form for your child (one form per child).
- Turn in the completed form to the Campus Principal.
The campus will contact you to set up a time to review your request.
