Welcome to the Fort Sam Houston ISD Parental Rights and Resource Center. This centralized portal is designed to keep families fully informed and in control of their child's educational, medical, and counseling pathways. Below, you will find mandatory Texas Education Agency (TEA) disclosures, health enrollment permissions, and our community-driven Title I engagement pacts.

School Health, Counseling & Medical Consents

 Texas law requires active parental consent before students receive specific non-emergency health or counseling services. Ensure your child's student profile is updated.
 

⚠️ Important Compliance Notice: Signed consent forms must be on file with your campus front office or clinic before services can be formally scheduled.

  • Notice and Consent for School Health-Related ServicesAuthorize routine clinical evaluations, specialized screenings, or telehealth support programs.

  •  Required Student Immunizations & Health ChartsDownload the mandatory minimum vaccine tracking rules required by the State of Texas for active public school attendance. 

    • Students who are not up to date with the state mandated immunizations will not be able to start school in the Fort Sam Houston Independent School District. Parents should ensure that all shots are taken care of now … well before the start of school. Click below to view the immunization requirements. 

    • For more information on required vaccinations, you may contact either of our school nurses. The FSH Elementary School nurse can be contacted by phone at (210)368-8840. The Cole Middle and High School nurse at (210)368-8746. 

    • Flu shot clinic information is provided by the San Antonio Metro Health District. Please visit their web site or call (210) 207–8750 for more information.
